Immature granulocytes (IG) are young, still-developing white blood cells (mainly precursors of neutrophils) that normally stay inside your bone marrow. In healthy adults, very few reach the bloodstream, so a normal result is close to zero: roughly 0.00 to 0.05 x10³/µL for the absolute count and about 0.0 to 0.5% of your white blood cells. A rising IG level usually signals your marrow is releasing cells early, often because of infection or inflammation.

What are immature granulocytes?
Granulocytes are a family of white blood cells that includes neutrophils, eosinophils, and basophils. They start as young precursor cells in your bone marrow and mature through stages before entering your blood. Immature granulocytes (IG) are these earlier developmental forms, mainly promyelocytes, myelocytes, and metamyelocytes. In most modern complete blood count (CBC) analyzers, the IG parameter groups these late-stage precursors together, but it does not include band neutrophils, which are counted separately.
Normally these cells stay in the marrow until they finish developing. When your body needs to fight an infection quickly, the marrow can push them into circulation early, which is why a measurable IG count often points to an active immune response.
What is a normal immature granulocyte range?
In healthy adults, immature granulocytes are usually absent or present only in trace amounts. A typical normal absolute count is about 0.00 to 0.05 x10³/µL, and the percentage version (IG%) is generally 0.0 to 0.5% of your total white blood cells. Because these cells belong in the marrow, seeing zero is completely normal and not a cause for concern.
Reference ranges differ depending on the hematology analyzer your lab uses. Some automated systems set a slightly higher adult upper limit, around 0.6 to 0.9%, and pediatric or newborn ranges can run higher still. This is why the number on your report may not exactly match the figures above.
IG is a relatively newer CBC parameter, reported automatically by many analyzers rather than counted by hand. It is best read alongside your total white blood cell count and neutrophil values rather than in isolation. A single mildly elevated result, without symptoms or other abnormal counts, is often repeated before any conclusions are drawn.
What a high immature granulocyte count means
A high immature granulocyte count means your bone marrow is releasing young white blood cells into your bloodstream earlier than usual. This is often called a left shift and is most commonly a normal response to your body ramping up its defenses.
Common causes include bacterial or other acute infections (including sepsis in more severe cases), inflammation, tissue injury, surgery, or burns, physical stress on the body including recovery from illness, pregnancy, use of certain medications such as corticosteroids or growth factors that stimulate the marrow, and less commonly bone marrow disorders including some leukemias and myeloproliferative conditions.
The size of the increase matters. Small rises are often seen with everyday infections, while larger or persistent elevations, especially alongside a very high or very low white blood cell count, may prompt closer evaluation. Elevated IG has been studied as an early marker of infection and sepsis, which is one reason clinicians pay attention to it.
There are no symptoms from the IG level itself. Any symptoms you notice come from the underlying cause, such as fever, chills, fatigue, or signs of infection. If your IG is high, follow up with your clinician, particularly if you feel unwell, have a fever, or if the result stays elevated on a repeat test. Your clinician will interpret it together with the rest of your CBC.
What a low or zero immature granulocyte count means
For immature granulocytes, a low or zero result is normal and expected. These cells are supposed to stay in your bone marrow while they mature, so finding none of them in your blood is exactly what you would hope to see in a healthy person. Unlike many biomarkers, there is no meaningful too low for IG.
Because the normal range already starts at zero, a result of 0.00 x10³/µL or 0.0% does not indicate a deficiency or a problem. It simply means no early granulocytes were detected in the sample. There is nothing to correct or treat.
If your overall white blood cell count is low, your clinician will focus on that number and the specific cell types involved rather than on the absence of immature granulocytes, which remains a normal finding.
What affects your immature granulocyte levels
Several everyday and clinical factors can nudge your IG level up: active or recent infections (the most common driver), inflammation from injury, surgery, or chronic conditions, physical or physiological stress including intense illness and pregnancy, certain medications especially corticosteroids and marrow-stimulating drugs, and recent vaccination or acute immune activation.
Age also plays a role, since newborns and young infants can normally have higher IG values than adults. Lab-to-lab differences in analyzers and reference ranges affect the reported number too. Because IG responds quickly to short-term events like a passing infection, a single result reflects a moment in time and is best interpreted in context.

Immature granulocytes are reported as part of the white blood cell differential within a complete blood count (CBC). The differential breaks your white cells into types and developmental stages, so IG is read alongside related measures such as band neutrophils, metamyelocytes, and absolute neutrophils. Looking at these together gives a fuller picture of how your marrow is producing and releasing white blood cells, which is far more informative than any single value on its own.
How to support healthy immature granulocyte levels
Immature granulocytes are not a marker you manage directly. Because they mostly rise in response to infection, inflammation, or stress, the goal is supporting general immune and marrow health rather than targeting a number. Broadly protective habits include eating a balanced diet with adequate protein and micronutrients, staying up to date on recommended vaccinations, practicing good hygiene to reduce infections, getting enough sleep, and managing chronic conditions with your clinician.
If your IG was elevated, the most useful step is usually treating the underlying cause, for example resolving an infection, rather than trying to change the IG value itself. Retesting after you recover often shows the level returning to normal on its own.
